Medocity Launches New Provider Solution for Chronic Care Management

PARSIPPANY, NJ – As of January 2017, the Center for Medicare & Medicaid Services (CMS) will begin reimbursing physicians for a greater number of complex and non-complex codes under its expanded Chronic Care Management (CCM) program. Medocity has responded by launching its new CCM software and services enabling care providers to significantly increase revenues and earnings.

Medocity offers two options for practices, provider organizations and ACOs who want to participate in CCM:

Full Service
Medocity’s clinical team will perform monthly, non-face-to-face care management services under a provider’s general supervision. Core services include medication reconciliation, patient education, reminders, scheduling referrals and patient follow-up. In addition, Medocity satisfies CMS requirements to:

  • Ensure 24-hour access to clinical staff
  • Create and maintain shareable electronic Care Plans
  • Facilitate care coordination and transitions of care
  • Document and track billable time and qualifying activities

Technology Only
Recognizing that some providers would prefer to perform these services themselves, Medocity will also license its cloud-based technology on a standalone basis, featuring sharable electronic care plans, secure messaging and televisits, activity tracking, billing reports, patient care summaries, medication management and custom resources.
